Welcome to Humber River Family Health Team
Humber River Family Health Team (HRFHT) offers a wide variety of FREE clinical services to our patients.
Services are delivered in partnership with Doctors, Nurse Practitioners, Registered Nurses, Dieticians, Social Workers, Chiropodist, Pharmacist and Administrative Support Staff.
We offer Free group education health classes to anyone in the community. You do not need to be a patient of the Humber River Family Health Team to attend these health promotion classes.

After Hours Clinic Information Line: 647-288-1006. Virtual consultation with the Doctor On-Call is available to all patients registered with the FHT & FHO for urgent sick visits only.
